At York Hospital, our care management team comprises social workers and RNs who work together to assist patients and families with social, emotional, and financial needs. Our team collaborates with patients, families, and all members of the patient care team to provide an optimal experience while they’re an inpatient or in the emergency room.
Prepared for the care you need
The nurses and social workers provide information, referral counseling, crisis intervention, care management and discharge planning services. Based on physician orders, our team arranges for such things as home care, hospice, medical equipment, insurance companies and other assistance. We’re here for you!

Advance care planning
An advance care plan provides instructions about the healthcare you want (as an adult 18 years of age or older) if you become too hurt or ill to speak for yourself. We offer a downloadable advance directive form as well as a downloadable reference guide to assist you if you’re trying to navigate the process of documenting final wishes.
